You must:

Fax or email your bank statement for the credit card used in this order.
We need the statement the bank sends you each month and can not be
hand-written or typed.

Your bank statement must include:
The cardholder\'s name
The billing address
The last four (4) digits of the credit card number


You may blank out any other information. Please be sure to include your
order number on your statement. We will ship
your order once all of the correct information has been reviewed.
We must hear back from you within three business days or we will be
cancelling your order.
